SPORTS PHYSICALS: Charlo’s Logan Health Mobile Unit is offering sports physicals on June 24 and July 28 from 9:00 AM to 3:00 PM at the school.

Appointments will be scheduled at three students per hour—please come prepared with:

• Pages 1–2 of the MHSA sports physical form fully completed
• A brief explanation for any “yes” answers
• The back page signed by a parent/guardian
• A contact number

Cost is $30, payable by cash or check at the time of the appointment. Slots are first come, first served.

The Logan Health Mobile Unit is now available on campus! It will be located in front of the district office at Charlo School every Monday from 9am to 4pm. The mobile unit can provide physicals and general health care services to students, staff, and community members.

Appointments can be made by calling 406-607-8187. The unit will be available through the end of the school year and throughout the summer.

Please note: This is like any doctor’s appointment—students must be checked out by a parent and have permission.
